The Rural Health Transformation Program (RHTP), authorized by the One Big Beautiful Bill Act (OBBBA), is a $50 billion federal initiative designed to fundamentally reshape and revitalize healthcare access, quality, and outcomes across rural America.
This program allocates $10 Billion annually to approved States from Fiscal Year 2026 through 2030. While only states can apply, this funding is intended to support innovative models, infrastructure improvements, and care coordination through partnerships with providers like yours.
RHTP Key Strategic Goals & How Behavioral Health Aligns
Improve disease prevention, chronic disease management, and behavioral health outcomes using evidence-based, outcomes-driven interventions.
You deliver it: Integrated care, MAT for SUD, crisis response, and preventative mental health services.
What's Considered Rural?Help rural providers become long-term access points by improving efficiency and financial sustainability.
You provide it: Cost-effective, community-based care that reduces costly ER visits and hospital stays.
Attract and retain a high-skilled healthcare workforce in rural and underserved communities.
You build it: Successful recruitment/retention models (e.g., CCBHCs) and training for specialized staff (e.g., peers, SUD counselors).
Foster the use of innovative technologies that promote efficient care delivery and patient access to digital health tools.
You use it: Telehealth, remote monitoring, health information exchanges (HIEs), and cybersecurity platforms.

Send the letter to your state’s RHTP contact or relevant state agency director. Follow up with a phone call or email to offer your services, data, and insights to strengthen the state’s application.
Explicitly let your state know how your agency is already meeting RHTP statutory objectives, such as:
- Behavioral Health Integration (co-location with PCPs)
- Care Coordination (reducing hospital transfers)
- Rural Outreach (mobile units, community health workers)

State-by-State Funding Variability
The RHT Program funding is unique because half of the total allocation is based on the quality and content of the state’s plan, which considers factors like rural population and the proportion of rural health facilities.
This means each state has significant discretion over how the RHTP is structured, managed, and who receives funding (sub-awards). A governor may designate a Department of Health, a Department of Human Services, or another lead agency to submit the application.
Your engagement with the correct state RHTP contact is essential to ensure behavioral health is not overlooked.
Expert Tip: Check with your Governor’s office or state health department immediately to identify the specific RHTP contact or lead agency responsible for developing and submitting the state application. Time is running out to shape their final proposal!
