CANS
Children and Adolescent Needs and Strengths (CANS) is a multi-purpose tool utilized to support clinical decision making, level of care, service planning, and monitoring of clinical outcomes. Multiple versions of CANS are currently used across all 50 states in mental health, juvenile justice, and early intervention arenas to name a few. ASAM
American Society of Addiction Medicine (ASAM) criteria are integrated into Qualifacts’ systems to provide a holistic, biopsychosocial assessment utilized for service and treatment planning as well as level of care determination. Our ASAM assessment integration can be used to determine the appropriate level of care for covered Substance Use Disorder (SUD) treatment services.
PHQ9
The Patient Health Questionnaire (PHQ-9) is a multi-purpose tool utilized for the screening, diagnosing, and monitoring of depression severity. It can be used to assist the clinician in making the diagnosis of depression and to quantify depression symptoms and monitor severity. Qualifacts easily integrates PHQ-9s into our systems to streamline the form collection process.
DLA-20
The Daily Living Activities-20 (DLA-20) is a functional behavioral health assessment tool used to measure how a person is managing everyday life tasks over the past 30 days. It assesses 20 areas of daily functioning, linking mental health symptoms or substance use to the person’s ability to perform essential life tasks. Qualifacts has integrated DLA-20 to provide valuable data that drives service delivery and tracks clinical outcomes.
Columbia Suicide Severity Rating Scale (C-SSRS)
The Columbia-Suicide Severity Rating Scale (C-SSRS) is a trusted, evidence-based screening tool that helps quickly identify individuals at risk of suicide. Its brief, structured questionnaire (often just 2-6 questions) makes it easy for staff to use consistently and accurately. By guiding clinicians to ask clear, direct questions about thoughts, plans, intent, and past behaviors, the C-SSRS improves compliance and confidence in risk assessments. This proven approach ensures agencies can efficiently determine who needs support and the appropriate level of care—helping keep clients safer while streamlining workflows.
GAD-7
The Generalized Anxiety Disorder 7 (GAD-7) instrument is a short, self-administered questionnaire designed to screen for generalized anxiety disorder (GAD) and assess its severity. The GAD-7 consists of 7 items asking how often in the past 2 weeks the respondent has been bothered by symptoms such as feeling nervous, excessive worrying, restlessness, trouble relaxing, irritability, and fear of something awful happening. Although designed for GAD, it also has utility as a general anxiety screener and shows reasonable performance in detecting other anxiety disorders (e.g. panic disorder, social anxiety, PTSD) in some settings.

The Standardized Behavioral Health Assessment covers the required domains and allows for more free-text areas with data elements for tracking outcomes. Several industry-standard screening tools and assessments are available (WHODAS, Socrates, AUDIT, ANSA, PCS-35, etc.) in the system.
