Mandatory Attestation Disclosure:

A provider does have to sign a contract when purchasing Qualifacts Systems’ Carelogic Enterprise S2. The terms for contractual obligation are negotiable and include all third party add ons. Our certified product is comprised of the Carelogic S2 core system and a few tightly integrated third party systems that we describe in this Mandatory Disclosure Attestation document.

Carelogic Enterprise S2 and its integrated DrFirst Rcopia application shall require a monthly fee per EP to support (§170.314(b)(3) /§170.314(a)(10)/ §170.314(a)(2) §170.314(a)(15)) Electronic Prescribing, Drug Formulary Checks, Drug-drug, drug-allergy interaction checks, Patient Specific Education Resources.

Carelogic Enterprise S2 and its integrated Emdeon Clinician Exchange Labs application shall require a monthly fee per EP to support the electronic (§170.314(b)(5)) Incorporate Laboratory test and values/results.

Carelogic Enterprise S2 shall require a monthly fee per mailbox for MaxMD DIRECT messaging which is used for (§170.314(e)(1)/ §170.314(e)(3)/ §170.314(b)(1) & (b)(2)  View, Download, and Transmit to 3rd Party, Secure Messaging, Transitions of care: – receive, display, and incorporate transition of care/referral summaries., Transitions of care: – create and transmit transition of care/referral summaries.

Carelogic Enterprise S2 shall require a one-time cost to establish interfaces for reporting to §170.314(f)(2)/ §170.314(f)(3) Transmission to immunization registries, Transmission to public health agencies – syndromic surveillance, and specialized registries.

CareLogic Enterprise S2 may charge the user a fee based on utilization per customer agency per month for data storage space for documents in the document library used with (§170.314(b)(1), §170.314(b) (4), §170.314(a)(12)) Transitions of care: – receive, display, and incorporate transition of care/referral summaries, Enable a user to electronically reconcile the data that represent a patient’s active medication, problem, and medication allergy list., Image results.

Carelogic Enterprise S2 may charge a monthly fee per user for the use of CareLogic S2 integrated IMPACT Assessment tool if the user elects to use this tool for §170.314(c)(1)-(2) Clinical Quality Measures – capture and export, Clinical Quality measures-import and calculate.

Carelogic Enterprise S2 may charge a monthly fee per report writer for the use of the ad hoc reporting tool using third party software Pentaho for use with ( §170.314(a)(14), §170.314(g)(2)) Patient Lists and Patient Reminders, Automate measure calculation.

Carelogic Enterprise S2 recommend MS Healthvault as its patient portal for use with (§170.314(e)(1)/ §170.314(e)(3)/ §170.314(b)(1) & (b)(2)  View, Download, and Transmit to 3rd Party, Secure Messaging, Transitions of care: – receive, display, and incorporate transition of care/referral summaries., Transitions of care: – create and transmit transition of care/referral summaries. We charge no fee to our users and there is no fee for the patients for this third party tool but they must use this tool for the criteria cited above.

Carelogic Enterprise S2 has a license and includes the “Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition. ™ Copyright ©2013 content from the American Psychiatric Association.” The American Psychiatric Association requires users to gain permission and Qualifacts Systems, Inc. collects a fee for each user of this content. Therefore, Carelogic Enterprise S2 may charge a monthly fee for the use of this Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition diagnosis content which is used for the following criteria (§170.314(a)(8), §170.314(a)(14), §170.314(a)(15), §170.314(b)(2), §170.314(a)(5), §170.314(b)(4), §170.314(e)(2), §170.314(f)(3), §170.314(c)(1)-(3), §170.314(b)(7)
